Verifications are formal declarations by which one swears to or affirms to the truth of the statements in a document. Verifications also include a statement of a Notary Public that the person appearing before the Notary has been properly identified as being the person purported to be appearing. A Verification is a cross between an Acknowledgement and a Jurat.
(1) The signer appears before the notary in person with a signed document or signs the document in front of the Notary
(2) The Notary then administers an Oath asking the signer "Do you swear or Affirm that the stements in this document are true to the best of your knowledge. The Signer must verbally answer with a "Yes", and a gesture such as a nod is not acceptable.
(3) The Notary completes a Notarial certificate by fixing their Seal and Signature
